WHO WE ARE
We like to think of ourselves as an outpatient physical therapy clinic that provides treatment in the home setting. Our physical therapists spend on average 60 minutes one-on-one time with each patient visit. Treatment is provided by a licensed physical therapist, no assistants or aides.
CONDITIONS WE TREAT
At Smart Rehab, we treat all physical conditions related to aging, with a focus on balance and fall prevention. We conduct a thorough analysis of each patient’s conditions and needs, then tailor the therapy to assure that each patient achieves their maximum potential.
WHO IS ELIGIBLE
Any Medicare eligible patient with a need for physical therapy is appropriate for the services we provide. We are not a home health agency so the patient does not need to be homebound in order to receive our service. Please call Smart Rehab with any questions to see if you qualify.
