The whole idea

One patient at a time, where they actually live.

Therapy happens in the home you intend to stay in, with one licensed therapist for the full hour. That single decision is what makes the care better, and it is the same in every market we work in.

  1. The evaluation

A licensed therapist comes to the house and evaluates you there, then writes the plan of care and sends it to your doctor to sign. You do not need a referral before you call.

  2. The visit

Sixty minutes, one patient, one therapist, start to finish. The same therapist stays with you for the whole course of care rather than whoever is free that day.

  3. The disciplines

Physical, occupational, and speech therapy are all delivered by one organization, so a second need does not mean starting again with a second company.

What most people get wrong

3 reasons people don't reach out for services.

Tap each one. Most people believe at least 1.

What we treat

Three disciplines, all delivered in the home.

Physical therapy

Balance and falls, strength after a hospital stay, walking distance, stairs, joint replacement recovery, Parkinson's, chronic pain. The work happens on your actual stairs, in your actual bathroom, in the shoes you actually wear.

Occupational therapy

Dressing, bathing, cooking, transfers, and the specific tasks that have started to take help. An occupational therapist looks at the task and the room together, because the room is half the problem.

Speech therapy

Swallowing, speech clarity, voice, and cognitive-communication after a stroke or with dementia. Swallowing is assessed with what you actually eat, at your own table.

Coverage questions

What Medicare covers in Kentucky.

Is in-home therapy covered by Medicare Part B in Northern Kentucky?

Usually, yes. Part B covers outpatient therapy delivered in the home on the same basis as therapy delivered in a clinic, and Smart Rehab is enrolled as a Part B provider. Coverage turns on medical necessity, so where you are treated does not change whether the benefit applies.

Must I be homebound before you can treat me?

No. That condition attaches to the home health benefit, not to Part B outpatient therapy, and mixing the two up is the single most common reason people here assume they are not eligible. You can drive across the bridge to work and still qualify. Our Part A home health service, run with partner agencies, is the one with a homebound condition.

Do I have to see a doctor first?

No. Ring us and ask for an evaluation. A coordinator calls back to verify your coverage, the therapist arranges the visit with you directly, and the plan of care goes to your physician afterwards for approval and signature.

What will it cost me?

Medicare patients normally pay the standard Part B coinsurance once the yearly deductible is satisfied, and secondary cover often takes care of it. We verify the benefits first and give you the number before a visit is booked. Without insurance, the evaluation is $175 and each following visit is $125.

Are your therapists licensed in Kentucky?

Yes. Care in Northern Kentucky is delivered by physical, occupational, and speech therapists licensed to practise in Kentucky, working out of our Covington office.
