Who we are
Smart Rehab provides in-home physical, occupational, and speech therapy for older adults. Our office is at 111 N. Taylor Ave, Suite C, Kirkwood, Missouri 63122, and you can reach us at (314) 447-8511.
What this policy covers
This policy covers information collected through this website. It does not cover your medical record. If you become a patient, the health information we keep about your care is governed by our Notice of Privacy Practices under HIPAA, which is a separate document. Ask for a copy by calling the number above.
What you give us
We only collect what you type into a form. There are three.
The evaluation request asks for your first and last name, ZIP code, phone number, email address, and whatever you choose to write about your needs and concerns.
The referral form asks for your name, your relationship to the patient, your email address, a callback number, the patient's name, the patient's ZIP code, and a description of the needs or concerns.
The therapist application asks for your name, credential, the states you are licensed in, how much you want to work, your email address, and your phone number.
We do not ask for a Social Security number, an insurance number, or a payment method anywhere on this site.
Where it goes
Evaluation requests and referrals are delivered to a Google Workspace account that Smart Rehab controls, and they notify our founders by email. Google provides those services to us under a business associate agreement, which is the arrangement HIPAA requires before a vendor may handle health information on a provider's behalf.
What the site itself collects
This site stores one thing in your browser: which of our markets you last looked at, so the phone number shown to you is the local one. That value never leaves your device and you can clear it by clearing your browser data.
Typefaces are loaded from Google Fonts, which means Google receives your IP address when a page loads, the same as on most websites. Our hosting provider keeps standard server logs.
How long we keep it
Form submissions stay in our Google Workspace account as a record of your request. If you do not become a patient and you want your submission deleted, call or email us and we will delete it.
Your choices
You can ask what we hold about you, ask us to correct it, or ask us to delete it. If you are a patient, you have additional rights over your medical record, and those are described in the Notice of Privacy Practices rather than here.
Children
Our services are for adults. This site is not directed to children and we do not knowingly collect information from anyone under 13.
Changes
If we change this policy we will change the date at the top of the page.
